Microsoft acknowledged the issues related to Microsoft Forms. Microsoft Forms is Down for some of the users or tenants. The users are unable to share Microsoft office forms after creating the survey.
Many users are complaining on Twitter about the Microsoft Forms issue. When you finish creating the form and click on ‘Collect Responses’ to send the forms out to the users, then it freezes Edge and then ‘This page isn’t responding.’
Microsoft 365 team confirmed that there is an issue, and they are investigating the issue related to MS Forms. If you are impacted normally, you would be able to see an incident FM419115 in the admin center.
Many users on Twitter shared that they can reproduce the issue, but there is no notification in the Admin center portal. We, HTMD, tested the MS Forms creation process in our tenant and are not impacted.
Update 6 PM UTC 24th Aug – The services are restored. The recent update to the service resulted in an unexpected impact on the Forms service.

Microsoft Forms is Down and Unable to share Microsoft Office Forms
You can login to forms.office.com and create the survey. We don’t see any reports on this part of the MS Forms services. This part works just fine for all the users/tenants.
However, when you try to share the MS Office Forms survey from impacted tenants using the Collect Responses button, the entire browser is getting frozen. It seems the link or URL for the forms survey is also not getting generated.
Microsoft Feedback on Unable to share Microsoft Office Forms
Microsoft acknowledged that there is an issue with MS Office Forms services. More details are available in the incident ticket FM419115. HTMD teams didn’t have access to the ticket information. Let us know if you have more details on the issue.
Microsoft said on Twitter that “We’re investigating an issue that matches your description. Please look for FM419115 in the admin center for more details.”
Another update around 2:00 PM UTC (24th Aug 2022) from Microsoft 365 team on Twitter is that “We’re investigating an issue where users are experiencing unexpected crashes or freezing when using the Microsoft Forms service. For more details please look for FM419115 in the admin center.“
